There may be additional charges (for example, pre-procedure office visits, surgeon, anesthesiologist, radiologist, pathologist, consulting physicians, emergency room physicians, supplies, and drugs). Charges are applied based on the practitioner's orders and services provided.

The estimated cost is not a guarantee of insurance coverage. Consumers are encouraged to check with their insurance company to understand the best benefits and out-of-pocket responsibility for the service chosen.
